68 year old female with Altered sensorium since 2 days

This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. 

Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs.

CHIEF COMPLANTS:-
Patient was brought to the casualty with the c/o altered sensorium since 2 days

HISTORY OF PRESENT ILLNESS:-
Patient was apparently asymptomatic 2weeks back then she developed vomitings 5 episodes which is non bilious,non projectile,food particles as a content associated with nausea treated with iv fluids at local hospital.
Loss of appetite,generalised weakness from 2 days,altered sensorium worsened,intermittently gaining Orientation
H/o burning micturition
No h/o fever,loose stools
No h/o constipation
No h/o chest pain, palpitations,sob
No h/o seizures,loss of consciousness and headache.

PAST HISTORY:-
K/c/o HTN since 20 yrs on Tab.telmisartan 40mg+clinidipine+chlorothiazide 12.5mg
K/c/o DM since 20 yrs on Tab.glimiperide 1mg +metformin 500mg
